When you are part of SQCN, you have access to our care management team of registered nurses, pharmacists, health coaches, social workers, and diabetes educators. This team will assist your practice with better help for your patients including:

• Coaching and education to address chronic diseases like diabetes, hypertension, and asthma
• Identifying the need for preventive care, like breast and colon screenings, diabetic testing, and immunizations- and making the appointments
• Following up after hospital or emergency room visits to ensure your patients understand after-visit instructions, have medications, and upcoming visits, if needed
• Community resources for SDOH needs
