The Sentara Quality Care Network is an organization of over 2,800 providers in the Hampton Roads, Charlottesville and Rockingham regions, Sentara Healthcare (including Sentara Martha Jefferson Hospital and Sentara Rockingham Memorial Hospital) and Children’s Hospital of the Kings Daughters, taking accountability for providing more effective healthcare for populations of patients. The network is an agent for improving your patients’ health and managing the total cost of care for services provided by aligning the right care, at the right time, and in the right setting.
The Sentara Quality Care Network came together in 2013 to offer a new solution and an alternative to the managed care model that has been dominant for more than ten years.
The network is developing new standards for accountability by enabling providers to collaborate to achieve a best practice alliance of clinical knowledge, technology and skill that will make healthcare easier to deliver and less costly – without sacrificing autonomy or patient satisfaction.
How Will Providers Benefit from Being a Member of the Sentara Quality Care Network (SQCN)?
More effective care creates value – for patients, payers and providers. SQCN raises the performance bar for providers through the integration of clinical and employer objectives. Physicians are able to utilize SQCN resources to expand their practices’ capabilities while achieving meaningful and measureable results.
- Additional care management resources and other healthcare professionals are made available to and within participating physician practices to assist with the management of chronic disease, other complex medical conditions, or choices that result in high costs that could be effectively managed in a lower cost setting.
- A secure clinical repository portal allows providers timely and accurate clinical data in order to improve the coordination of care and to identify care that is needed yet not received.
